If you need to make a report using the Hazing Reporting Form, please email Student Conduct & Conflict Resolution (SCCR@ufsa.ufl.edu). 

Please provide as much detail as possible including the names and contact information for the involved individuals, the name of the organization(s) involved, when the incident occurred, where it occurred, a brief narrative describing what happened, and whether anyone was injured and/or alcohol was involved.
